How this practice collects, uses, and protects your information. Last updated: [date].
01Summary
Sessions are held by secure video only.
This website collects only what you enter to book or send a message.
Your information is never sold or used for advertising.
Therapy records are protected by federal law (HIPAA) and Illinois law, which is often stricter.
Confidentiality has a few legal limits, listed below.
02Telehealth
All sessions are held by secure video, so you can attend from anywhere in Illinois without a commute or waiting room.
Illinois law requires you to be physically located in Illinois during each session. If you'll be traveling, let me know ahead of time.
Video sessions use [video platform], a HIPAA-compliant service that has signed a Business Associate Agreement with this practice.
03Information this website collects
When you send a message, the site collects:
Your name and email address
Your phone number, if provided
Your message
Confirmation that you agreed to this policy
When you book a consultation, you enter your name, email, and chosen time on Proton Calendar's booking page, and Proton shares them with the practice.
Your browser also shares basic technical information, such as your IP address, with the web host for security. This site does not use advertising cookies, tracking pixels, or analytics.
04Please don't send health information through this site
Website forms and regular email are suitable for scheduling only. Please share only what's needed to set up a time. We'll discuss clinical details in session.
05How your information is used
To respond to you and schedule appointments
To provide therapy and keep required records
To bill your insurance, if you choose to use it
To meet legal and licensing requirements
Your information is never sold, rented, or shared for marketing.
06How your information is stored
Consultations are booked on Proton Calendar's encrypted scheduling page, which Proton operates under its own privacy policy. Messages sent through this website go to the practice's encrypted Proton Mail account. The website itself does not keep a copy.
Clinical records are kept in [records system], a HIPAA-compliant electronic health record.
Messages from people who do not become clients are deleted within [90] days.
07Your clinical records
I do not share your records, session content, or personal information without your written consent, except as described under Limits of confidentiality.
The Illinois Mental Health and Developmental Disabilities Confidentiality Act often protects therapy records more strictly than federal law. A subpoena alone is not enough to release them. In most court cases, a judge must review the records privately before ordering their release.
08Insurance
If you use insurance, your insurer receives only what's needed to process a claim, usually a diagnosis code, dates of service, and session type. Insurers do not receive session notes. Private pay keeps your care out of insurance records.
09Limits of confidentiality
As a licensed clinician in Illinois, I am required or permitted by law to share information without your consent in these situations:
Suspected abuse or neglect of a child
Suspected abuse, neglect, or financial exploitation of an older adult or an adult with a disability
A serious and imminent risk to your life
A specific threat of violence against an identifiable person. I may warn that person and notify police.
A clear and present danger to yourself or others. Illinois requires certain clinicians to report this to the Illinois Department of Human Services within 24 hours, for firearm (FOID) eligibility only.
A court order issued after a judge reviews the records
Mandated reporting is a debated policy. It can protect people, and it can also harm families who are already heavily monitored by other systems. I'm required to follow the law, and I'll be transparent about it so you can make informed choices about what you share.
If you're unsure whether something would require a report, ask me first. Discussing your own feelings, history, and needs is different from sharing current, identifying details about a child or vulnerable adult at risk.
